Insurance
We promise to work with you to maximize your insurance benefits so you can get your dental care completed in a timely manner. If you need additional financial assistance to supplement your insurance contribution, we offer CareCredit patient financing. Dental PPOs contribute to the cost of your dental care. Dental PPOs do not cover all of the dental procedures that you may need or that you might want. For example, many cosmetic dental procedures are not covered by Dental PPOs.
In-network With Dental PPOs
We are in-network with dental PPO plans. Our staff will prepare your PPO insurance claim form and process it for you. We will only ask for your PPO insurance estimated portion when you have dental treatment at Lincolnwood Family Dental. Most of the dental procedures will require a co-payment. Your estimated co-payment will be due at the time your treatment is rendered. We appreciate your cooperation.

Yes, you can provide this when you make your first appointment with us, and the office will call you prior if you don’t have it at the time when you call to make your first appointment. Make sure to bring any medical records ahead of time and a list of medication you are currently taking.

Do you have payment plans available?
